NEW PATIENT INFORMATION

We are pleased that you chose Pain Consultants at Piedmont for the diagnosis, treatment and management of your pain. To acquaint you with our office policies, we provide the following information.

Initial Consultation

Your referring physician must fax or email results of pertinent studies (i.e. MRI, EMG) and documentation of any previous care to our office. If we do not receive this information in a timely manner, we may need to reschedule your appointment.

Procedures

If you are scheduled for a procedure (even for an injection), please bring a designated driver to transport you home. If you are unable to arrange for a driver, please let us know so we may assist in your transportation.

Welcome to the Pain Consultant's Patient Portal

The Patient Portal is an online communication link between patients and their providers. Equipped with a secure username and password, you gain anytime access to your Medical Health Records from the comfort of your home or office 24×7.

INSURANCE/AUTHORIZATIONS

We accept most insurance programs and their subsidiaries. If we do not participate in your insurance plan, please speak to our office manager, Tye McClellan at tmcclellan@paincon.net to find out if other arrangements are possible. If your insurance requires prior authorization for a visit to a specialist, you must also obtain this authorization number prior to your visit. Our office staff is available to assist you with this process if necessary. Contact us by phone or send us a message.

PAYMENT POLICY

Insurance co-payments are due at the time of arrival. We accept cash, checks, and credit cards.
